Our well-child visits for your children are important to ensure necessary preventative care, track your child’s growth and development, and establish a strong relationship with us as your pediatrician. GROWTH & NUTRITION PROGRAM

The goal of the Growth and Nutrition Program (GNP) is to offer focused medical care to infants and children who are struggling to gain weight.  These growth problems happen for a variety of reasons. Our team will help figure out the root cause of the child’s growth problem and get them where they need to be on their growth curve.

Eligibility Requirements: 

• Children ages 6 and younger
• Difficulty eating or feeding
• Losing weight or not gaining weight.

The Growth and Nutrition Program at Caring Health Center features an interdisciplinary team that includes a pediatrician, nurse, lactation consultant, nutritionist, social worker, and community health worker. This team coordinates care with specialty providers and community programs like WIC, to provide culturally tailored and holistic care for your child and family.  We are experienced in working with patients of diverse cultural backgrounds and income levels. Ask your child’s provider for a referral!

Growth & Nutrition Team Evaluation Process

• Clinical assessment by a Pediatrician

• Intake visit with the Pediatric Care Coordinator

• Social risk assessment, connection to community resources, and home visit
by a Community Health Worker

• Care plan support with the Nurse Care Manager

•  Nutrition visits and nutrition group classes with a cooking component 

•  Meeting with the Pediatric Social Worker 

•  Coordination of care with specialty providers and referrals as needed
