All employees hired to work thirty (30) or more hours per week are eligible for coverage under a comprehensive package of medical care benefits. Employees have the option to select from HMO or PPO plans. Coverage for employees is effective thirty (30) days after date of hire. Employees electing coverage are required to pay a share of the premium cost on a pre-tax basis.
Any employee regularly scheduled to work thirty (30) or more hours per week is eligible for coverage under a comprehensive package of dental care benefits. Coverage is effective thirty (30) days after date of hire. Employees electing coverage are required to pay a share of the premium cost on a pre-tax basis.
Any employee regularly scheduled to work thirty (30) or more hours per week is eligible for coverage under a comprehensive package of vision are benefits. Coverage is effective thirty (30) days after date of hire. Employees electing coverage are required to pay the cost of the premium on a pre-tax basis.
Eligible employees may enroll each year in reimbursement accounts for medical expenses and for dependent care expenses. The employee funds their own accounts with pre-tax dollars through payroll deductions subject to limitations.
Any employee regularly scheduled to work thirty (30) or more hours per week is eligible for coverage under the health Center’s group term life insurance plan. Coverage for employees is effective (30) days after date of hire. In the event of an employee’s death while employed, his/her designated beneficiary will receive a benefit equal to the employee’s annual base salary rounded up to the next $1,000 to a maximum benefit of $120,000 thousand. The premium cost for this coverage is fully paid by Caring Health Center Inc.
Long-term disability insurance is provided to eligible employees (30 hours or more) at the expense of the health center. Coverage is effective thirty (30) days after date of hire of employment. After 180 days of total or partial disability, the plan pays 60% of base salary to a maximum of $6,000 per month.
Paid sick time is provided to all employees, accrual starts on the hire date. Employees hired for 20 or more hours accrue up to twelve (12) days each year. Intermittent and per diem employees accrue up to forty (40) hours per year. Accrued sick time is not payable upon termination of employment.
All full time (40 hours) and part time (20 hours or more) employees are entitled to 12 paid holidays per year. This includes one (1) floating holiday which is earned after (1) year of service. Part-time employees will earn prorated hours based on their scheduled hours.
Full-time and part-time employees, employed for at least one year, earn 16 hours of personal time on July 1st each year. Employees employed for at least 6 months earn 8 hours of personal time.
Vacation time is provided to all employees regularly scheduled to work twenty (20) or more hours per week. Time is accrued on a weekly basis after thirty (30) days employment. Earned vacation time cannot be used until after 6 months of employment. The amount of time that accrues is dependent on the position and length of service with the health center.
All employees can contribute towards an employer sponsored 403(B) retirement plan. CHC offers up to a 2% match with a 3-year vesting period. A Roth option is also available.
